Every year, any student under the age of 18 must have they’re parents or guardians fill out the Release of Liability. For any student wishing to participate in any activity sponsored by Calvary Chapel Placerville on site or off site, this form must be completely filled out and submitted with updated information before coming to the event. Also, for each separate event, an addendum to the release must be signed to acknowledge that the student has parental permission to participate in the event.
Also below is a student information card. If any student new to the ministry wishes to be contacted or would like more information, they may fill it out and submit it directly to the office.
